As a condition of participation in the Sand City Volleyball Club, the undersigned consents to the capture of their own image and that of their child/ward through photography and video. The undersigned further agrees that all such materials shall become the property of Sand City Volleyball Club & may be used, without further notice, review, or compensation, in any publication or media format for the purpose of advertising, promotion, or publicizing the Club and its related activities.

The remaining balance will be split into equal monthly installments, due on the first of each month from September through December. Participants who elect to utilize the payment plan shall be subject to an additional $200 administrative fee.
