As a condition of participation in the Sand City Volleyball Club, the undersigned consents to the capture of their own image and that of their child/ward through photography and video. The undersigned further agrees that all such materials shall become the property of Sand City Volleyball Club & may be used, without further notice, review, or compensation, in any publication or media format for the purpose of advertising, promotion, or publicizing the Club and its related activities.
